Each state will be required to sign a License Agreement with infoUSA.

First paragraph: Enter the date your agency signs the contract, your agency name and addres. |

Section 2.c: Enter your agency's website address |

Section 5.b: Enter the required information for the person to whom infoUSA will mail the products. |
|
Section 9: Enter the required information for the person to whom any notices are to be sent. |
|
Section 12: Enter the signature block information under LICENCEE. |
Send one (1) signed original of the License Agreement (pages 34 through 44) directly to infoUSA at the address shown below and a signed electronic copy will be returned to you via email. If you require a signed original, then please send two (2) signed copies to infoUSA. infoUSA will forward a copy to Iowa for their records.
